By registering for this event and checking the box below, I acknowledge and understand the following:
1. Assumption of Risks: Participation in the DJ Sackmann Training LLC basketball training camp carries inherent risks, including the potential for physical injuries. Some of these injuries can occasionally be serious or even life-threatening. I recognize and accept all risks associated with my child's participation in the event.
2. Release from Liability: I, as the parent or guardian of the participant, hereby release and agree to hold harmless DJ Sackmann Training LLC, its owner Donald Stephen Sackmann Jr., and all affiliated trainers, coaches, volunteers, and staff from any claims, liabilities, damages, or rights of action, present or future, whether the result of injuries, negligence or any other cause arising from or related to my child's participation in the camp.
3. Medical Authorization: Should my child require medical attention while attending the camp, I grant permission to DJ Sackmann Training LLC staff and their designated agents to provide necessary medical care.
4. Proof of Insurance: I confirm that my child is currently covered by active medical insurance that will remain in force for the duration of the camp.
5. Media Consent: I consent to the taking and use of photographs and/or videos of my child by DJ Sackmann Training LLC. I grant DJ Sackmann Training LLC permission to use such images for promotional, marketing, and other related purposes, including but not limited to publications, advertisements, brochures, and their website.
